Model of Interdisciplinary Team in the Primary Health Care System while working with families at risk

Abstract

The article analyzes a model of interdisciplinary team for social risk families in the primary health care system from the point of view of family physicians, nurses and social workers. The data was collected using the focus group method – three different professional groups were created. Seeking to unfold a model of team aspects of team members, sharing of responsibilities and team leadership are analyzed in the article.
